NHS Somerset and Somerset County Council currently provide a range of services which support rehabilitation and reablement. They are currently looking at ways if improving what people gain from rehabilitation and using resources effectively. The aim is to look at ensuring that future services reflects the changing needs of the population and looks at the whole approach.
